When you've entered your information and reviewed all details please download the order using the "Export Form" button and email it to us at info@vclookfly.com or directly to your sales rep.

Special Note on Customizations:
- Leave Name / Number / Name2/Title fields blank if not required.
- Only include customization fields shown on proofs (eg., if shorts only have a number, don't enter a name as well).
- If you would like your name set differently than the proof shows, you must request it in the Special Notes section.
